Bleeding incidents while on HD (father)

My father has been on HD for about 3 years and he is close to 70 but the last 6 months have been in and out of the hospital because of bleeding in the brain. This first happened in November when I got a call and had to fly to india for surgery. They had to perform surgery to control the bleeding and in February they put the flap back only to have another surgery in April because of bleeding again. He has been on Heparin all this while and we have now started getting dialysis done without Heparin after this last surgery.

I was wondering if folks have suggestions on how to manage this situation going forward.
